This privacy policy covers Meridian Medical Associates LLC and its subsidiaries including (collectively, “Meridian Medical,” “we” “us” or “our”). We provide primary care with weight management and pain treatment and related services through our website, our branded applications for mobile and connected devices.

By using our services, you accept this privacy policy, which is part of our Terms and Conditions. We may modify this privacy policy from time to time. Meridian Medical may collect two (2) types of information about you: personal and non-personal.

Privacy Policy for SMS Text Messaging

This Privacy Policy explains how the Meridain Medical collects, uses, discloses, and safeguards your personal information when you interact with our SMS Text Message service. By using our service, you consent to the practices described in this Privacy Policy.

Information We Collect

1.1. Personal Information: When you subscribe to our SMS messaging service, we collect certain personal information, such as your mobile phone number, name and contact information to provide you with our services.
1.2. SMS Interactions: We collect information about your interactions with our SMS messages, such as the content of the messages you send or receive, and the date and time of such messages.
1.3. Device Information: We may automatically collect information about the mobile device used to receive SMS messages, including device type, operating system, and mobile network information. PERSONAL INFORMATION WE COLLECT ABOUT YOU (PIPEDA)

According to the federal personal information protection and electronic documents act, 2000 (PIPEDA), “personal information” means information about an identifiable individual. The information you provide to Meridian Medical is considered personal information it is possible to relate it back to you through your name, home address, e-mail address, telephone number and any other information that can identify you. If information cannot be related to an identifiable individual it is considered anonymous information (such as laboratory results identified by alpha-numeric identifier instead of individual’s name). Whenever possible, Meridian Medical works with anonymous information. This privacy statement does not apply to anonymous information.

TYPES OF DATA WE COLLECT FROM YOU

Personally Identifiable Information (PII). We acquire PII that may include, in certain contexts, your name, home address, payment information, email address, and telephone number. We also acquire your IP address, User ID, and/or Device ID, which certain jurisdictions consider to be PII because it could be used to identify an individual or Device if it were combined with other identifying information.

Anonymous Data. Some of the Data that we acquire cannot identify, distinguish or trace you or your Device, even if combined with other identifying information, and some Data that could be considered PII when combined with other identifying information is not used in a way that identifies, distinguishes or traces you or your Device, but is instead used in an anonymous way, often aggregated with other anonymous Data about other users.

You may submit data to us for limited purposes such as requesting customer support; answering a questionnaire; participating in a study; entering contests or sweepstakes; or signing up to receive communications from us.

PAYMENT PROCESSING AND PAYMENT PROCESSOR

We may provide paid products and/or services within the Website. In that case, we use third-party services for payment processing (e.g. payment processors).

We will not store or collect your payment card details. That information is provided directly to our third-party payment processors whose use of your personal information is governed by their Privacy Policy. These payment processors adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, Mastercard, American Express and Discover. HIPAA COMPLIANCE

The operation of Meridian Medical is in compliance with the Health Insurance Portability and Accountability Act (HIPAA).  And in accordance with HIPAA security rule, it is our legal duty to safeguard your protected health information (PHI). The information shall only be accessible by an authorized personnel having unique access rights.

Protected health information (PHI) and electronic protected health information (ePHI) are backed up and are recoverable. The information can also be permanently deleted when no longer needed by Meridian Medical.

The member of staff will either have authority to settle your complaint or will have ready access to someone who has the authority. Our response will fully address the subject matter of your complaint and, if appropriate, will offer redress. If you phone us during our investigation and the member of staff handling your complaint is not available, then another member of our team will try to assist you.

Unless applicable data protection laws require responses within shorter timescales, we will try to resolve any privacy complaints you have within 15 business days of receiving your complaint and in exceptional circumstances, within 35 business days (and we will let you know if this is the case).
